Patient Participation Group
Are you interested in finding out more about Church Street Surgery?
Would you like to influence the development of local health services?
The surgery is keen to set up a Patient Participation Group and we are seeking volunteers to help shape the future of the surgery. We are going to start by looking at two key issues:
- The appointments system - we have been working closely with NHS Wakefield District and Productive Primary Care to develop a new system from the beginning of April 2012. We would very much like your thoughts and ideas about these proposed changes so that we can develop a system that will meet the needs of everyone
- A new way of working with patients to help support those with long term conditions called care planning. Agreeing a care plan with a health or social care professional means being able to talk about their condition, how it affects their life, what they want to do and what more patients can do for themselves with the right support. The idea is that this will help our patients manage better and keep their condition well under control. We need patients to look at our plans, the information we plan to send out to make sure it is appropriate and clear. We are going to start by looking at care planning for patients with asthma, COPD, epilepsy, strokes, coronary heart disease and diabetes. If you have any of these long term conditions and can spare some time we would very much appreciate your input.
We would like to use this as a starting point and hope to work with you to build a really positive relationship. The PPG is not a channel for complaints. There is a system in place for dealing with any complaints you may have, details of which you can read here http://tempuri.org/tempuri.html.
How you can help or get involved
You can help in one of two ways:
- Volunteer some time to join our new Patient Participation Group.
We are looking to form a group of approximately 20 people that is representative of our practice patients. The group will meet 1-2 times a year.
- Volunteer some time to complete our patient survey.
This will be at least once a year and can be either postal or on-line to suit you.
