Empowering you, your friends and family to self-care may have many benefits for your short-term and long-term health, this is particularly important as people are living longer. Helping people to look after their own health, and their family’s health also helps to manage demand on health services.

Introducing the Appointment Guide
We (along side the Practice Health Champions) are introducing the appointment guide to help you make your 10 minute consultation as productive as possible, we hope that by doing this you and your clinician will get the best possible outcomes from the consultation.
We hope that this will help you have more control over your health and will ultimately improve your health, and the experience you have when you come to the Practice. By completing the questions it means that clinicians are able to use the time they have more appropriately improving health care outcomes for you.
These will soon be available and we would appreciate it if you could complete one prior to your appointment.
You don’t have to wait for a doctor’s appointment, you can go straight to your pharmacy.
Pharmacies in West Wakefield offer a service called Pharmacy First to give you easier access when you need health advice and treatment.
Your local pharmacy can give you advice and, if needed, medicines for common illnesses.
Your pharmacist is a qualified health care professional who can help with your health problems. The pharmacist may offer you a private space to talk to you about your symptoms.
If you don’t normally pay for your prescriptions, then any medicines supplied by your pharmacist will be free.
If you do pay for your prescriptions, then you will get free advice from the pharmacist and the cost of the medicines should be less than the prescription charge.

Why is your blood pressure (BP) so important?
When your heart beats, it pumps blood round your body to give it the energy and oxygen it needs. As the blood moves, it pushes against the sides of the blood vessels. The strength of this pushing is your blood pressure. If your blood pressure is too high, it puts extra strain on your arteries and your heart. Regularly monitoring your blood pressure helps us look after you - it can complement other tests which will help give us the bigger picture about your health.
How to monitor your BP
You can now use our waiting room BP machine any time during opening hours to monitor your BP. Just pop in, if you need some help just ask at reception and one of our specially trained care navigators will be happy to help you (please remember to write your name and date of birth on the ticket and hand it to reception).
Alternatively if you monitor your blood pressure at home using a home blood pressure machine please ring us on 01924 217 999 and select option 1 for enquiries. It would be appreciated if you could do this in the afternoon when it’s a bit quieter. Alternatively, if you have access to our online services you can send us your readings electronically logging on to SystmOnline (use the link at the bottom of the page or click here) then select the questionnaires section.
Friends and Family Test
From the 1st December 2014 all GP practices are required to ask patients the Friends & Family test questions that will be monitored by NHS England on a monthly basis. The two questions are as follows:
- How likely are you to recommend our GP Surgery to friends and family if they needed similar care or treatment?
- Which services would you least like to lose?
The PPG asked us to include the second question because the Practice is due to lose 13% of its funding over the next 4 years - this would mean the loss of up to two doctors or a doctor and several nurses and as such we would not be able to provide the current level of service that you receive.
It is important to us that you let us know what services you don't need so that we can structure our remaining service to meet your needs. We will continue to work with local MPs such as Mary Creagh to stop this happening as we are experiencing greater demand than ever for our services and we are concerned that it will result in more A&E attendances or walk-in centre appointments which will be detrimental to the NHS due to the cost. This is at a time when the NHS is already financially stretched.
Each A&E appointment costs between £56 and £245 even if you leave before you are seen (usually it is around £156), a walk-in centre appointment is £63 and a GP appointment is £25. We therefore feel that we provide a cost-effective service to the NHS and are very concerned that our service will be reduced which will be detrimental to all of our patients.
